PGR Ethical Review Form

All postgraduate research students on MPhil/ PhD course are required to complete the ethical review form linked above.

The form then needs to be provided to the student's upgrade panel and supervisors before the student's upgrade meeting.

The form is then completed at the student's upgrade meeting, including the outcome sections being completed and all parties signing the form.

Please email the fully completed copy of the form to pghistoryoffice@warwick.ac.uk

If further approval from HSSREC is required, please also email this approval to pghistoryoffice@warwick.ac.uk
